This crash have happened twice already:

Initially, I have my AMD CPU undervolt with an offset of -0.100. After the first crash, I have change the offset to -0.0875. Both crashes did not happen on the same day, but both did happen while in Apex.

From google, this error seems to happen when AMD CPU did not get enough juice when its trying to boost.

 

Any clue how to fix this? (Really not looking forward to stock setting, AMD's 1.5V single core peak is too much.)

If you're trying to save yourself some voltage while maintaining functionality, you should be also adjusting your PBO settings or disabling it then if not adjusting your clocks entirely. I'm also not sure what chip you're running, which would be useful as you could potentially use 1usmus's ClockTuner tool if you absolutely insist on undervolting your chip while maintaining stability.
