Front Panel USB Ports Not Working

I've had my build for about a year and a half but, for about a month now, nothing that I insert into my front panel USB ports are recognized. Things are sometimes recognized for a very brief period following a restart, but not always. I've mostly been testing the ports with flash drives that work just fine on my laptop. I've also tried plugging my phone in and it does begin to charge, but I get a message to check the connection and it still doesn't show up for data transfers. If it's a hardware issue, how would I tell if the issue is with my MOBO or my case?

 

What I Have Tried

- Checked that cable is securely attached to USB header on motherboard

- Disable / Re-enabling ports in Device Manager

- Uninstalling ports in Device Manager and repairing on restart
