Hello Guys,

I want to create a survey with 5 question with 3 different answers each ( Yes, No, Other ).

Option 1) If answer is "No" or "Other", user should be able to upload image,video,pdf

Option 2) User must answer these questions and can upload image,video,pdf

 

I have done it via Google Forms , but on photo uploading they have to sign in to google to upload it. I don't like this option.

Also responded answers are fit in 1 CSV file, I would like to get them individually in excel or word or something else for each user with attached image,video,pdf.

 

I don't know if this is crazy, but if you can help me or suggest I would be glad.

This will look like a report, these users have to fill survey till starting to work.

There is about 50 user, so each user might can fill this survey twice or three or even four times a day before starting to work.

File upload is very risky to build securely. You never know what hidden code the PDF could have (if it is even a PDF in the first place).

 

I don't think Google Forms is the tool for you then. Have you looked into Typeform?

Yes I know it is risky, but we are going to use this between our employees, no one else.

I said that I don't want to have Sign in to upload files, this is cause some of employees are far from computers :D I want to make it easier for them.

I will check TypeForm, thank you.
