Dead graphics card

My grapicscard might be dead. I have lived with artifacts on my screen for well over 4 years and random crashes where lines cover the screen. A restart usually fixes it but it reappears. Today it suffered the same thing and did this at startup and never booted, but instead just gave me a black screen, I can successfully boot into safe mode and enter BIOS, but not regular windows, any ideas or is my card dead for good this time?

1st thing i wouuld check is if the PSU has enough power, cus thats what happens if it doesnt.

The PSU is 750 W and i well above what the system needs and is should not have a problem with drawing power from outlet if that's what you are suggesting

750Watt means nothing if the PSU is garbage quality. But if its reputable brand and a good model yes its weird indeed.

I would check the cables, maybe reseat the connectors. (also the PCIe one). Maybe thats helps. 

Else examine the graphics card and see if you can see something wrong. Other than that, i dont know. Sorry.
