Hello, I have a Nvidia GTX 750, it is kinda old, I know, but now it started doing weird stuff, I plug it in the computer, it works just fine, i boot up the PC, but after 5 minutes it's gone, the resolution changes like I don't have the driver installed and it starts showing the desktop in multiple colors and random colors all over the screen, more like a rainbow. Is it dead?

Does it do that after reboots?

if its good upon boots, try cleaning it out and making sure its free of dust, then go download DDU and run it. Could be a driver issue rather than a dead GPU. If that doesnt fix it, then yes, seems like your GPU may be on its last legs.

After i reboot it works fine again for a few minutes and then starts that again, i opened it up already and cleaned it up and checked if anything is burned or something, it all seems good. And in those 5 minutes of it working just fine, games work perfectly, high fps, but after that they start having 5-10 fps

Go download DDU then and run that and re-install drivers. If it continues after that, then most likely a dead GPU.
