I had this issue with a disk drive - I fixed it by replacing the SATA cable with a brand new one.

If you can't be bothered buying a new SATA cable, try swapping the SATA port.  Also make sure the SATA cable and power is plugged in properly and not being pinched anywhere inside your case.

Sorry for the super late respond, however, It turned out that it was dying and I got a RMA on it.
