Artifacting?

A friend of mine sent me this image and asked for help. I'm terrible at troubleshooting so I was hoping you guys could help. I am working on extracting specs from him but iirc he has a 1060 3gb and a Hasswell cpu but I could be mixing friends up lol. Any help I can give him off the image, if you need more info I will ask him.

Yeah if he/she hasn't OCed at all then it's a faulty card

he's had it since Christmas so it wouldn't just start artifacting 6mo later unless its faulty. And he wasn't overclocking. He also cannot "fully load windows"
