Command timeouts ( Dying drive? )

I've decided today i need to speed up my system and took alot at cleaning and maintaining my HDD i came across a few things that i assume are a problem but would like further advice on this, 

188    Command Timeout                         OK            100           96            0             648550154422    N/A    

  5    Reallocated Sector Count                OK            98            98            36            1816          N/A    

 

I've uploaded the Doc with the rest of the HDD info if anyone would care to take a look
