HELP! Cannot Fomat USB Drive

My 16GB cannot be formatted it says write protected
and cannot be opened it says H:\ is no accessible

the file and directories is corrupted and unreadable

 

I try chkdsk on command prompt still no luck

the type file system is exFAT

Windows cannot run disk checking on this volume because it is protected

@GarnetDevil i was clear readonly on diskpart (detail on image) but still no luck, still cannot format because write protected.
