No signal problem

When i start the pc it works ~ 2 minutes and then the monitor black out and tell me "no signal". What could it be?

Specs:

Amd athlon 64x2 4200+

His Radeon hd 4650 1gb

Msi k9n neo v3

Psu 500w noname 12v 21A

overheating?

the fan isnt spinning?

faulty GPU?

Cpu temps are fine , i mounted a case fan on the gpu to cool it down since the fan for gpu is broken , it should cooldown better than before , when

Edit: Same thing on other psu , strange thing , when i put a finger to the back of the card it heats up soo fast and then it shuts down itself . How i fix that.?
