Captive Portal

I am looking for a simple captive portal solution, which users need to to put in their E-mail and accept the terms of the use.

 

Any one have any idea's?

I've never had to setup a Captive Portal but you could try using something like CoovaChilli (guide). If you don't want to have to setup something like this Ubiquiti does have integrated captive portals on their APs (you do need their controller software running for it to work though).
