Troubleshooting R7 260X

I've been having problems with my XFX R7 260x that I bought last year. Sometimes my screen will black out and my PC will automatically restart. But when I check the temps, the GPU is barely in the 50's. What's wrong?

Could be failing memory, failing core, bad psu, do you have anything overclocked? What are your full specs?
