Graphic Kerfuffle

Ok so I've been having various random crashes and graphical glitches ever since switching main displays.  I won't try go into to much detail, but this has happened a couple of times.  My 3rd display blanks and the other two do this.  I figured it could be a bad cable as my second monitor just didn't want to work sometimes.  I chalked it up to a bad cable as whether on DP on the gpu or HDMI on the iGPU it would simply not turn on at times.  I bought a brand new "16k" DP cable to put it back on the GPU but it's still glitching as pictured.

 

As Pictured it is SUW DP, UW DP, UW DP portrait.

 

It has been acting strange ever since switching the SUW instead of a 16x9 4k.  I can't give direct accounts for what it does, rather I have been shutting down app's that could cause issues like a fan program I can't remember the name of, signal rgb and wallpaper engine.  All unneeded and want to disable for trouble shooting.

 

Maybe it's my 2nd UW display going out sending a bad signal?  I will troubleshoot more and try to narrow issues, just want to put this out there for documentation and see if anyone has thought's

It has been a variety of glitches, most recently the past couple day's what is pictured.

Have you already reseated the GPU and all signal cable connections?  GPU power cable (both ends if modular PSU)?

glitches you showing here are usually vram or gpu die itself being the problem.

I just sent a support ticket into XFX incase this is the issue.

 

Last night I dropped down to just one monitor and today it black screened in a game for the first time, previously it's only been desktop related.
