Graphics card heating issue

Hi 
Attaching my PC confing 

My graphics card freezes in 15 to 20 min while gaming

i tried to trouble shoot via some tools, attaching screenshot of the logs as well,

just want to confirm if there is way to optimise the graphics card

or 

i have to change my graphics card,, if yes which one should i opt

Have you installed fresh drivers?
