Something feels wrong with my SSD

So I've had my computer for almost a year now, and just last week my SSD has been acting up. I'll be playing apex, or PoE and it'll desync me from the server, and just boot me. The reason I think it's my SSD is because of my Crystal Mark scores. Any help would be SUPER appreciated.
