Do you use a water cooler for your CPU ? if that is the case check for leakages(get a piece of toilet paper gently touch along the surface and see if it absorbed any moisture do the same for the pipes )  maybe a drop fell down on your GPU and now it vaporized and the GPU works ok again. 

No I use an air cooler, Dark Rock Pro 4.

Happened again, went away after a reboot. It started happening right after getting into the Windows 10 login screen (black squares). They were also appearing on the desktop (they weren't visible on a screenshot) but also had artifacts in Firefox that were visible. Why does this happen so rarely? It's been 2 months since it first happened.

I'll probably reinstall Windows 10 someday, I used to have a B450 Tomahawk MAX, 3600X and a 2060 Super on this current installation and over time changed all 3 of them (B550 Tomahawk, 5800X and 3070) without reinstalling it. But I doubt it will fix it.
