Why is this only under 3d settings? Does it only apply to 3d?

Also, if I have adaptive V-Sync on, do I need to enable v-sync in the game?

It works on any title and 3d settings is not the 3d your thinking of, no you do not need to turn vsync on in game

Games are considered 3D applications, this does not mean the 'glasses' 3D in this context, hence the '3D settings' menu being where you find the setting.

Adaptive V-Sync is turned on from that menu only, you want to turn the Adaptive V-Sync on inside your 3D settings menu (either global or you can make seperate profiles for individual games from the same tab) and leave the V-Sync setting in your game OFF.
