USB random disconnect

Very quick one, back on my old rig after some bad luck, old 8350 based system, had weird issues with usb devices for a while but its getting worse and starting to drive me up the wall, usb 3 does not work at all, I plug a device in and windows does the boop sound to say its plugged in but it never detects, usb 2 works at normal speed however randomly everything will disconnect at once, sometimes it comes back after about a second and sometimes it stays dead, replugging the devices in does not reset them and I have to power off the system to get them back, do we think this is windows messing up or a usb controller issue, honestly thinking of doing a clean install and starting it again.

not for the most part, the SD card reader I have has a couple usb ports on it too but the only thing plugged into that is the SD card and its only connected when I need it.
