m65 all the way. Its the only mouse I can actually play well with in FPS games. as for the mouse mat, Id go with the Qck or an mm200 from corsair. If you prefer a hard mouse mat, then pick a mm600

For FPS, all the mice you listed are not optimal. For FPS, you want an optical mouse such as Logitech G400s, Corsair Raptor M40/M45, SteelSeries Rival or Razer DeathAdder 2013. All laser mice have slight inherent acceleration, which means you won't get constant results. The amount of acceleration is low so it might not be noticeable if you don't look for it, though.

Also, DPI is just a marketing gimmick. Higher DPI does NOT mean better. It only means it can go faster than lower DPI mice. For 1080p resolution most people won't even need more than 3200 DPI, and many "professionals" use less than 1000 DPI.

As for mouse pads, SteelSeries QCK is great if you want something cheap that gets the job done, and the 4HD/9HD is great if you want something without friction. SteelSeries HD series really makes using a mouse effortless, but the surface can scratch the mouse feet.
