Random Black Screens

When i play games, mostly minecraft, dirt 3, destiny 2 and some other games, my screen randomly goes black for around 1-3 sec (sometimes up to 5), and i dont know whats causing it. Ive already tried updating/reinstalling my gpu drivers (running an xfx rs rx 480 4gb) but it still happens. I those 3 games i mention its at its worst in dirt 3, when it can go around 10-30 sec between before another black screen appears. I havent overclocked my gpu tho i have undervolted it (to keep it cool). Ive also Overclocked my monitor, from ~60hz to ~75hz. I personally think its the screens OC that makes this happen, but i aint sure.

Go to stock settings, if needed tighting the cables and see what it does. 

If possible, try a different connector

If you can test with another monitor or gpu then that would be great.
