Moonlight - Unknown Device

My gaming PC (running Win 10) is being used to use Moonlight to stream my video games, it was functioning correctly until today. It originally listed my PC's name but now says "unknown" and I can no longer stream my games. 

 

I had followed these instructions with port forwards on: 

 

TCP: 47984, 47989, 48010
UDP: 47998, 47999, 48000, 48002, 48010

 

When selecting my other devices, it lists their names. Only my gaming PC is not being listed correctly anymore.

 

I can provide more info as needed.

I'd rather not factory reset yet because it may interfere with my housemates' PS4s' settings.

 

Using AT&T's Arris BGW210-700.

 

Streaming to iOS and MacOS Mojave.

 

Errors:

Connection Failed: control stream establishment failed with error -1

I assume you're using DHCP at home, so the IP address of one of the computers/devices has probably changed. Compare the IP addresses against whatever firewall / port forwarding you have done.
