Cacti and mactrack setup

I have setup cacti on Ubuntu and have installed the mactracker plugin. Problem is I can't get it to work with my cisco switches. Screenshots below show:

 

Bottom image - 7vwiS.png - The errors that are being shown, everything is unknown

Top image - yDzuM.png - The configuration page for a cisco 3750 switch

Middle image - aM9Cm.png - The device type that should correspond to the cisco 3750 switch

Please can someone tell me where I have gone wrong
