Find active connections for program

Hello, I'm trying to find active connections for a program. Is there a good program for this? or is there a way to get netstat to display only for 1 application?

what I need to find is the ip, what port it uses would make it better but not a must.

 

If this program can exclude ip from a list that would make it a hell of a lot easyer too, but this is also not a must.
