Underclock/overclock changed overnight

So I had my 390 undervolted and overclocked to keep the temperatures and noise levels down.

I was able to run it at +5 power and -45 mV at 1065/1625, this allowed me to keep the fans at 53% or lower - which is basically silent operation. Now I am running +5/+9 to achieve a 1050/1625 overclock, which made the card need upwards of 70% fan speed.

I checked HW Monitor and MSI afterburner and the core was hitting the overclock set before with the 'undervolted' card. Now the card gets horizontal lines at those same settings operating in just windows. Not sure what happened. I did some cable management and propped the card up to reduce sag (with rubber HDD mounts), but if it was actually getting less power, wouldn't it be cutting out in a more defined manner? I did not change anything, no BIOS settings, nothing. Only physically moved the card a cock hair. The card is functioning as you would expect other than needing more voltage.

I am either thinking the card was pushed too hard and now requires more voltage (I used to run a 1160/1650 OC at full power and voltage) or something bizarre is happening with power delivery. I did not update drivers from the time of this occurring but I did have to reinstall MSI afterburner because the previous version locked up at some point (guessing the OC failed?).
