Make sure you have Cloud Saving enabled, then Steam should automatically transfer your saves when you install. As for mods, those have to be reinstalled.

what do u mean copy your save files across to the new pc

If you look in your documents folder on your account (Windows), you should see a My Games folder, which should have your save files in it. I don't think the game saves are stored in the cloud (unless you specifically tell Steam to do so)

Find the folder with all the fallout game data in it and find the saves. Literally copy and paste them onto a thumb drive or something and tada!

The saves are usually separate from the game data. The game data is normally in Program Files, while saves tend to be somewhere in the User's folder. So normally: "C:/Users/*Username*/Documents/My Games/Fallout 4" or somewhere after "/Documents"
