I tested this fan controller outside of my build first, to make sure it was working; it was - no issues. I then unplugged unused fan header extension cables and the temperature sensors, then installed it in a 5.25" bay.

 

When I turned my computer on, the fans did not spin up. The LCD screen still turns on, and the temperature sensors (which i plugged back in after freaking out about maybe killing new hardware) displayed the ambient temperature. I noticed that there was glue holding the temperature sensor connectors in; the transistors below were not damaged in the glue's removal (if they were i doubt the unit would still function). Nothing else was touched and I was grounded during the installation.

 

The fan controller appears to no longer supply power to any fans connected to any of its 5 fan slots, as pressing the fan symbol (to change the speed of the currently selected fan) no longer responds.

The fans function normally on all of my motherboard's headers.

The same molex power connector was used before and after the loss of fan control, and for good measure I tried other ones.

 

Anyone have any insight into what the problem could be?

 

tl;dr Fan controller worked fine outside of computer, stopped powering fans after installation.

If you bought it recently I would try to get a replacement. Have you made sure the pins on the molex were plugged in correctly? They are badly made, a pin could be pushed in. I had that issue recently but at the point you would get 0 power at all, and since your screen turns on...
