I have a random black screen that sorts itself out after some time.

Things first went wrong when I tried to connect a HDD to get some files off of it. This resulted in some odd power cycling which eventually sorted itself after disconnecting it. Not too long after that Windows did not recognize the card and AMD Adrenaline would not launch. I started to get corrupted display drivers after every restart despite reinstalling them a few times. I fixed it with DDU and driver install. Several days later I was met with a black screen. Checked connections, swapped display cables but nothing worked. Popped in my old HD 5450 and it booted. Put the RX 580 back in and nothing. Got an RMA on the GPU but I thought I would try it one more time and it worked. In the last week it has black screened 4 times. Two of them just required a restart while the others I had to leave it alone until the next day before it worked again. Two of these happened while gaming and the other two just while staring at the Windows screen.

I am hesitant to send it off because if it tests okay for them I am out shipping and almost 2 weeks without a computer. I'm leaning towards the PSU because it is old and at the minimum requirements for the GPU so maybe it can't quite keep up anymore and I just have been lucky for the last three years. Maybe the HDD stressed it just enough. Any thoughts what is going on?

Hard to say for sure what the issue is, but corrupted files would be a good place to start troubleshooting.  I had an SSD give up about a year ago with similar symptoms.  You can run the system file checker that comes with Windows and that will detect and (hopefully) fix the issue.  If it detects something wrong and the issue persists then it's probably a failing drive.
