Need mouse recommendation

I think I'll hitch onto this thread instead of making another one of those mouse rec threads.

 

The only place I can stand having buttons is near the scroll wheel. I'm usually limited to 3 buttons thanks to that - my hand reaches and grips all over the place. Anyone know of a decent mouse that isn't as tame as a basic optical mouse?
