If you have system restore points, I'm pretty sure that's the reason.

 

I personally use Defraggler (replaced the Windows one) because it's much more detailed and tells you every file and the destination of it.

 

The one I can never defrag is "SystemVolumeInformation" which is my system restore points. I kept only the latest one and had 1% fragmentation. (I used to have 4% when I had 2 restore points).

Even defraggler can't defrag it. Find your restore points and remove them. Keep the last one just in case, even though most of the time people don't need them. 

 

It's Windows 7 but I'm sure the programs are named the same. http://windows.microsoft.com/en-ca/windows/turn-system-restore-on-off#1TC=windows-7

 

Don't worry too much about it, only at around 5% will you get a performance decrease, 10% is noticeable.
