Sound Issues I am puzzled.

I have a Behringer Q802 USB mixer, XLR mic hooked up and a pair of headphones connected via a 1/4 inch to 3.5mm y connector. So my issue is my sound and words are very low quality now, out of no where. I have to plug the 1/4 inch plug like halfway for the sound and words to sound decent, but still not as good as normal. I have tried different USB ports, a 1/4 adapter that screws onto my 3.5mm headset plug does the same thing, and halfway plugged in its decent. I am lost and puzzled, any ideas? I also used air and blew out the 1/4 hole on the mixer and all connectors are clean.
