My screen keeps freezing and goes black for a few seconds

My screen randomly freezes then goes black for a few seconds 

When my screen comes back on radeon notification pops up and says 

Default Radeon WattMan settings have been restored due to unexpected system

My system 
CPU -- Intel Core i5‑6500 3.2 GHz Quad‑Core Processor SkyLake

GPU -- MSI Radeon RX 480 4GB GAMING X Video Card

RAM -- G.Skill Ripjaws V series 16GB

SSD -- PNY 240GB

PSU -- SeaSonic 520W 80+ Bronze Certified Fully-Modular ATX Power Supply

Mother Board -- ASUS B150 GAMING/AURA

Potential Driver Issue, possibly a Bad Card. I have the same issue on my HD 3850. Try amping up the fan-speed, heat may be the issue. If not you probably have a dead card.

I would first try reinstalling the latest drivers for your graphics card and see it that fixes this issue although if it does not then the most likely cause is going to be possible hardware level damage that has occurred to the card. In that case you can try to replace it yourself although that may not be easy and without guarantees or could send it off to a professional which would most likely charge a fee for their service although should get your card working again without the issue.
