PC shuts down while gaming or reboot loop happens and also a humming sound in all the usb ports

Hello.I have a 3 years old pc.I was using gtx 580 which was acting weird like if I update driver pc hanged.I had to hard reboot pressing the power button.An error showed every time that "display driver stopped responding and recovered successfully."I took it to repair center and they said bad card.So I bought a new one "Sapphire rx480 8gb last week.It updated nicely.First two days I played well without any problem.Recently reboot loop happened first.Then yesterday again reboot loop.Today it did not rebooted anymore but crashing while gaming some time.I can watch movies or keep this way whole day nothing wrong but when I start gaming it crashes after some time.I checked all the temps look good.But don't know why this is happening even in normal cpu temp like  60 degree.I have attached some monitor results.

 

And I also hear a humming sound in all my usb ports when I put usb cable of my headphone.Please help.

 

System Specs:

Processor:Core i7 2600k

Mobo: Asrock Fatal1ty pro p67

Gpu:Sapphire nitro rx480 8gb Ram: Corsair vangeance 8gb 1600 mhz

Cooler: I had corsair h100 liquid cooler but it died last week so I am using stock cooler now.

Psu: corsair 1200 gold (I got this psu as a gift from my brother abroad)

Casing: Corsair 600T

 

I also encountered some error events.Errors I found are attached in screen shots.Please ignore the power error in screen shot,that time it happened by load shedding.

anybody check the power supply while it was in the shop?

do you still have your old card?

were the old drivers uninstalled before the new card was installed?

how many different sound drivers are installed?

when is the last time you went to windows updates?

Its your power supply most likely gonna say 99.9% sure, Your +12v is really off by a lot. It should be sitting around 12.06 - 12.09 yours is at 11.352. It looks like your power supply is in need of replacing its failing and thus why your getting restarts while gaming.

If it only occurs while doing something intensive like gaming then most likely it's your power supply. Looks like it can't handle outputting enough power. Get it replaced asap.

Very sorry for late reply.I was a bit busy with work.

1.No they did not said anything about psu when checked thay said card wasitad

2.Yeas I still have my old card,I want to check it later if it can be repaired

3.I fully reinstalled windows so yes old driver uninstalled with it

4.Only one.e AMD Catalyst software suite is packaged with the driver.

5.Windows updated last week.
