Help My Computer Is Crashing And I Don't Know Why

ok as the title says my desktop PC is crashing. I have two monitors the 1st on the right and 2nd on the left. both are plugged into the GPU. the 2nd monitor is connected via a display port to hdmi cable, and it turning green when the PC crashes. the 1st is connected to the GPU with a normal hdmi cable and the screen turns black. all my audio devices stop working as well and they all are pugged directly into the mother board. when i re start my computer it runs perfectly fine until out of nowhere in game or not it crashes. some times it crashes many times in a row. I have tryed re seating all of my components and re pluging in all of my PSU cables to there designated spots. I also have tryed uninstalling, clearing, and reinstalling drivers. this fixed the problem for about 3 weeks until it happened again. this issue has been persisting from about 1-2 months. my pc parts are listed bellow(tbh idek if posting the parts will help or not but it cant hurt right?).
PC parts
AMD Ryzen 2600x
Stock AMD air cooler
AMD RX5700xt
16GB of DDR4 3200 T-Force ram
AsRock B450M-HDV
EVGA 700w Gold+
Kingstone 240GB SSD SATA 2.5" Boot Drive
1TB Seagate HDD
5 random fans i found on amazon
a thermal take case
2 1080p monitors. #1 is 75hrz and #2 is 60hrz
Please Anyone Help Me.

no overclocks on CPU, and i don't think xmp is on. how would i check if it was? bios?

Are your temps good and drivers up to date.

I have had a similar issue with a Vega 56. 

It was fixed with a clean install of the driver.

cpu temps according to a similar reddit thread i have running say are VERY bad 94.50C with a prime95 stress test of 15ish min. and yes i have a clean driver install

That's pretty hot. Could defiantly be a problem. Is your graphics card overheating too.

 no 5700xt under load is about 60C

you can also see if there is a bios update for the MB, as it can help improve CPU and Memory compatibility, go to bios and see what version you have right it down or take a picture of it and check on the MB websites like Asrocks website
