Im desperate on this issue. nvlddmkm crashes and graphics blocks

So I for over a year now have been having graphics crashes. Which at face value seems very simple. But I cant seem to fix it. What happens for the most part is that my screens will go black and sometimes recover but crash the game I am playing. or it will completely freeze up the computer and I will have to force shutdown and restart. Also sometimes instead of a crash like this. it will crash the game without screen crashes and say the application has been blocked from using my graphics hardware. But both of these issues seem to just team up and decide at random whose turn it is today as i cant find any consistency with them and anything.The issue also isn't consistant as to when it crashes. Sometimes it will be on lightweight games sometimes heavy games and vice versa. making it even harder to narrow down. When I look in the event viewer I get a few things that look like they could be the issue and I have googled them to my wits end and tried multiple fixes for each on to no avail. this issue even persists after starting windows fresh. So here are the following events I get in the viewer that might be of interest.

 

1: Event 219, Kernel-pnp. (The driver \Driver\WudfRd failed to load for the device HID\VID_B58E&PID_9E84&MI_03&Col02\7&1344f0d8&0&0001.)

2: Event 4101, Display (Display driver nvlddmkm stopped responding and has successfully recovered) -this is a lie. it usually hangs and doesn't recover. this is also the more common event

3. Event 4109, Display (Application has been blocked from accessing graphics hardware)

 

Here is a list of some of the things I have tried to do to fix these issues.

1: Simple reinstall,update,repair of graphics driver

2: Using display driver uninstaller to completely remove any remnants of the driver and reinstall them completely clean

3: Tweaking registry values  to try and trick the time out

     3a: TdrDdiDelay-20

     3b: TdrDelay-20

     3c: TdrLevel-0

4:Going into windows update and restarting the system fresh.

5: Wiping all  my drives  and reinstalling a fresh copy of  windows using  a usb

6: Checking  all connections on the gpu and power supply and making  sure the gpu is seated correctly in its slot

7: Underclocking the card as much as I can with evga precision x. Turning the offset to -90

 

 

These are just  the ones  I can remember off the top of my head. when it comes to registry I have tried a ton of stuff and honestly cant remember all that I may have changed in there over the course of all this.

Im at my wits end on this and I cannot think of anything else at all or where to even begin to narrow it down with all the inconsistency. I will work with anyone and provide any information that leads to fixing this. All I can ever find anywhere when I search for these  issues is the typical "update/reinstall  drivers" none of that has worked for me in the slightest.Well  I guess I should post my specs and hope for the best. Thank you for your time for anyone reading this and taking the time. Even if  you  don't have any solutions.

 

 

Motherboard: Gigabyte Ga-Z97X-UD5H-BK

Cpu: Intel Core I7 4790k

Ram: Corsair Vengeance 1866mhz DDR3 (CMY16GX3M2A1866C9)

Gpu: Evga Titan X Hybrid (the pascal one I believe. it was in the 900 family)

Psu: Cooler Master M2 silent pro 1000w

Ssd: Samsung 840 evo 250gb

Hdd: Wd Blue 1tb 7200rpm

         Toshiba 2tb 7200rpm

 

Keyboard: Logitech g910 orion spectrum

Mouse:  Razer naga 2016

Headset: Logitech g933

Im hoping its not the gpu. It was 1300 bucks when I bought it. But it was bought brand new from Newegg. also it already is underclocked as low as I can get it on evga precision. ive even lowered the power target to 60% and still have the issues. And im not sure if I could use it with generic drivers. cause then I couldn't use all of my screens. I understand the testing purposes but that wouldn't really be a solution to me personally. As to trying it in another pc we  don't own another that has enough power to run the card to even test it. and we cant afford a new card either. also it has 12gb of vram so I don't think tthats an issue unless maybe the speed is overclocked out of the box as well.

oh I misunderstood on the display driver. and screen thing. but I have tried that before and the issue still happened. because it would be when I had reset windows and started fresh and hadn't installed any nvidia drivers. Also there actually is no pc shop around me. except for geek squad and well nobody trusts them. As for the hardware monitor I will post the results here because im not sure exactly what to look for. I  see that on the mobo at the +12v it has a value of 12.024 with a minimum of 11.952. Then as to the gpu connecters it is an 8 and a 6 pin. and I have always had them on 2 separate cables. because I didn't think 1 cable would be enough nor did I have 1 cable that had enough expansions to fit both of the connections at once. so its 1 cable from the psu to the 8 pin and 1 cable from the psu to the 6 pin. To try with a single cable I would have to order one that would fit both at the same time.
