how do i use the pcpartpicker add custom part

It's for an item that isn't listed in the PCPartPicker database. You enter a price and it will add to the total. It's pretty helpful so see how much an entire setup costs, or if you need to buy an unlisted item with a build but don't want to forget it.

when I buy the system and I have a part that's not in the database and I don't have it will I get the part with the rest of the build

Assuming you added a link, and if you add it to the cart of where you're buying the parts, yes. PCPP just tells you the lowest price and at what retailer, you don't buy it directly from them.
