Help! Display goes nuts after returning from SLEEP mode

I have built a new machine with all the drivers installed properly but whenever this machine goes to sleep and returns back, the display goes nuts as shown in the image below. Please help!! I have tried to move the mouse, press keyboard buttons, replug the cables but nothing happens unless I shut power off and start the PC again. It's a small mini itx system.

Other System (Laptop)

i7 3632QM | Toshiba | 8GB Corsair VS RAM | Sandisk SSD PLUS | 500GB Toshiba HDD

I do leave the computer on many times and at those times I want the computer to calm down and also save some energy. It never happens on my laptop.

then use hibernate instead. sleep mode is broken ever since it was invented.
