The portion that does DC-to-DC conversion in power supplies is small compared to the rest of the power supply. Getting rid of 5V and 3.3V won't really do much for size or cost.

 

As an example, take a look at the original Xbox 360 power brick. It only provided 12V and up to ~200W of power.

That'll require a LONG time for adoption from multiple sides of the equation.
