Samsung Galaxy A5 2016 Battery Problem

Hey I'm having a peculiar issue whenever I plug in my Samsung Galaxy A5 2016. The phone is about a year old and when it's around 20% charge or lower and I plug it in the battery level plummets. For example it was reading as 17% a few minutes ago and i plugged it in. within a minute it was reading as 4%.

 

Anyone know 1. what could be causing this and 2. how i could resolve the issue?

Seconded, super common sign of battery failing, it will only get worse. Sometimes I hear a factory reset will improve the situation but it has never worked for me.
