rx 460 RX 460 getting screen tearing and blurry video

I'm running windows 8.1 64 bit. So here is the full story, one day Microsoft released the Creators update. After I got it the issues started, many others fixed the issue by disabling xbox features and game mode. I tried to get help but couldn't get any answers. I then got frustrated so I switched to windows 8.1. The issue STILL continues and I really need help. Some games are near unplayable with this issue. Everything just looks blurry. This only started happening when the creators update was released. HELP PLEASE

 

I contacted blizzard support and they told me something was wrong with my gpu but wouldn't tell what it was. Below I have attached files which to them showed some problem.

 I tried taking a picture but it looks normal on the picture.

I use the HP 23e and IDK what you mean settings

what cable are you using?

 

Is the res set to the native of the monitor?

Oh sorry I was gone yes its native 1080p and I'm connected by hdmi
