Low frames but I don't know why

I've had my pc almost for a year now and I was fine with it but I had a lot of scream tearing. I now use v-sync on and motion blur and medium settings. But the FPS is 30-50 and sometime skips to 12 .

my specs are below

intel i5 4460

gtx 970 zotac 4gb

h81m-s8m

8gb ram

.....

this should all be compatible and decent but I don't know what is wrong 

 

can you help me please?

what game? usage of cpu and gpu? temps of cpu and gpu?

use hw monitor and msi afterburner to get this info.

cpu=57 celcius (133 f)

gpu=74 celciu (165 f)

 running watch dogs 2 at medium settings

Wath Dogs 2 is extremely CPU intensive and even 4C/8T i7's can have problems with it.
