My Iphone 6s freezes a lot (What could be the issue?)

My phone would sometime freeze for 2 min then comeback. My screen won’t respond but I can still hear the audio playing if i’m watching a video or i’m on a call. It used to happen a couple of times during the day now it’s gotten annoying. 

 

Here’s an example in the video. (my screen froze around the 5sec mark and cameback at 54 sec)

What iOS version are you on and what percentage battery health?
