The worst thing I ever managed (other than installing a mainboard without the standoffs) was way back in 2000 when I installed a new SCSi controller in a file server.
Pin1 on the board wasn't marked properly so I installed the ribbon cable the wrong way around.

Powered the server and the little server room was immediately filled with white smoke. Killed power and opened the server again, the whole ribbon cable was just bare wire. Since SCSi used power on the cable, it instantly transformed the plastic covering the wires into smoke. Just the bare strands were sitting there and a VERY irate boss was standing behind me.

Luckily all started correctly with a new (rather expensive) SCSi cable and no damage to anything.
So we learn from our mistakes and hopefully laugh at the blunders along the way.
