G930 Product PID or S/N number ?

So my g930's cracked at the top of the left earphone and left it hanging in the cable. I opened a support ticked and they asked me to provide them with a couple of things.

 

But for some reason I can't find the product PID or S/N number.

 

Help please!

Also, I bought them at Amazon. Where do I find the receipt for them? Or proof of purchase? First and only time I ever bought anything off amazon.
