GTA V - very low fps outside of city on Gtx 980.

Nope, "well-optimized" for me would mean that the application uses the resources given fully to get the compute power required to work at optimal...but yeah that includes the fact that it must have the option to use multiple threads if available, which unfortunately not many games do. Most of the time the thread 0 will be used at 100%. Work should be divided throughout all threads. At least that is what DX12 promises:

In this situation, advanced graphics will not be such a cpu hog.

Except you are confused about what DX12 does.  GTA V uses all 4 cores to 100% EASILY.  DX12 eliminates draw-call bottlenecks that can happen, it doesn't automatically make DX12 games more CPU-efficient.  There is nothing Rockstar can do in the case of CPU-utilization when it's main issue is the API, DirectX.

There is a huge difference between "utilizing all cores" and "multi-threaded".  True multi-threading has not been invented yet in the programming world.

i have a gtx980 and am getting ~59fps and lovely appearance at 4k res with the following:

default "advanced graphics" (off)

no AA anything

"high" everything else including ambient occlusion & tessellation

"softest" shadows

max motion blur

CPU is over-clocked at 4.2GHz

there's an occasional stutter here and there but the "alt-tab" workaround seems to help.

overall, a fantastic game.

What if you turn everything to "Very High" and "Ultra" + Nvidia shadows only with no AA. Try that and tell me what u get. Also mention CPU load.

Hmm... That could help, thanks. There isn't a big difference between being on or off is there?
