Diagnose Graphics Card Best Way

Hi. I need to know if my Graphics Card is somehow faulty. What is the best way to check it?

Stress test it?

Could you be more specific? Which program? How long? 

When my monitor goes from windows to sleep mode, I can only wake it to the lighted black screen and i need to restart pc to be back again.

Play AAA titles.

I'm sick of playing games like BF4 so i need something to stress it for me and to be sure that if it will pass this test, that the card is fine. In every way.
