Did you remove it before you re-installed it?

then maybe use something like CCleaner, to completely remove it .. then install again

Well it worked for three shutdowns then the issue returned, thinking it could be a registry issue maybe?

can anyone offer some help, had the same blue-screen 5 times since the last post.

Can you give me a screenshot of the settings/properties menu and then the sounds tab (where you can enable and disable different sounds for different events. When you receive a call or a message etc and then take two or more screenshots of all those events so I know which events are checked and which are not checked. )

The reason for this is because I had a similar issue to this on my own Windows 8.1 computer a while ago and I solved it by disabling some/one event's (don't remember how many) sound effect. But I can't remember which. So if I can compare your list to mine I can tell you what to uncheck and you can see if that will work. At least it did so for me.

Do you have ICS (Internet Connection Sharing) turned on? If so, turn it off (it's under services.msc)

here is a screenshot: http://prntscr.com/4zvjos

I also just received a BSOD with this error code: driver_irql_not_less_or_equal

Do you have an overclock?

Disable the ones i marked with blue http://gyazo.com/ea7f9d973e2a1b067b22637e257c1307

And try uninstalling mcaffee with revo uninstaller.

Mcaffee sucks massive balls and often causes more problems then it does fix.

Use windows defender.
