RJ11 or RJ9 to 3.5mm + USB

What I want to do is to come out of a telephone handset jack (RJ9 or RJ11) and then into a 3.5mm AND a USB.

Ideally I would have an amlifier so I can control my own voice volume and the volume coming into my headset.

 

I want to connect a Bose quiet comfort 15 headphones and a high end mic maybe Blue yeti to the system for working on the phone in a small loud room.

 

What is out there that is possible? I am open to any thoughts or ideas.
