only happens on mc, and yes ive done a fresh install. keep in mind, this has only been happening ever since i refreshed windows.

try the minecraft forums they are more knowledgeable on minecraft.

I assume you've update your drivers and Java? Have you tried it without Optifine (Its a bit well broken from time to time)? What version of Minecraft is it? Have you changes your RAM allocation/permgen etc at all?

Edit: If your on a 64-Bit OS, make sure to manually get 64Bit java.

Java 8 is Sketchy, I would recommend Java 7 Build 67, Note: I play with about 50+ Mods, min most of the time.

If you have a Graphics card try checking the Control panel and seeing if anything is set up in a weird way for minecraft.
