Ideas for Cooling with 10mm Clearance

I'm playing around with a thin-ITX board.  There are 1U coolers than can cool a CPU with less than 34mm clearance.  There are not any 0.5U coolers than can cool a CPU with 10mm clearance.  Any ideas on how to achieve this?

 

My best guess so far is to use a blower fan and file down a 1U heat sink to height.  I'm hoping the collective think tank can create another solution.

I think most all-in-one cases come with cooling.

I can't find it now, but I saw a very low profile cpu cooler that could have been a little over 1cm tall. It was series of heatpipes moving heat to an offset low profile stack of aluminium fins, with a blower fan blowing air through them. While it might have been 1cm tall, it was also very wide horizontally, and likely wouldn't fit on most motherboards. I think it was from Intel as well.

This is 13.5mm tall but you can file it down.

PS: Yes I know it isn't meant for CPU's but I am sure you can find a way to mount it. Also get the lowest TDP CPU you can find :P

Would you all agree that a fan is a better idea than using the case as a radiator?  I feel that even a weak fan is way better than passive.
